Survey Solutions for the Web (Perseus.com) is a pretty good value. They have a great tool for one question surveys (free download) but also a more extensive package, including web, email and disk surveys. I think its $179, but they do have a fully functional 30 day trial software you can download. I did so almost 30 days ago, and found their software to be straightforward, and their technical support was excellent.
