Maximizing Response and Minimizing Bias

I thought that some of you might be interested in commenting on a web survey that I have designed. I would appreciate any feedback you would like to give but there are two aspects that I am wondering about. (Even though I think of them as separate, they are not entirely independent of one another)

1. I would like to know what bias might be associated with my survey questions. The online survey can be found at www.selkirkfinancial.com/survey.

2. I was wondering which methods would maximize response: mass mail-out, mail-out with follow up call, cold calling, mass faxes, etc.

The research is targeted at financial managers (CFO, VP Finance, controllers, treasurers, etc) of companies with revenues between $10 million and $150 million. As incentive, respondents will be entered into a draw for a PalmPilot organizer or weekend for two at Hilton Hotel (winner chooses). For added convenience, respondents have the option of responding by a phone survey, fax survey or the web survey with the above url.

Please note that the web site survey is not linked to our homepage and is not designed to get responses from "surfers". My thinking was that people might appreciate an option of submitting an online survey as opposed to phone or fax. In that sense it is different than "traditional" online surveys and is the reason why it is more extensive than you would think.(It also makes it easier for me to collect responses into a d-base)
